Does anyone have a good Games for Learning Design Template? Something that looks very official to give to a new client that will cover the objective of the game, timeline available, resources available, etc. 

I can create one, but wondering if someone has a good one that they use. Or what questions do you ask a new client that is asking for you to make them a game or activity for a workshop?

Here is an example of a write-up from HRDQ, http://www.hrdqstore.com/Marooned.html

It seems to cover the things you describe in a client facing way.You might want to go to their catalog and look at other examples. Their write ups all follow the same general outline.
